Time for Enrollment - APPLICATION DUE THIS WEEK

We need to get our application in this week - do we need to have a dinner tonight or tomorrow night to fill it out or does somebody want to man it alone?

    all i have left on the application is chemo's insurance stuff and a krewe name.

    i don't think the sketch matters that much, but i'll put something down
